Effective August 11, 2026
Do not submit sensitive information through the public form. Do not enter Social Security numbers, account or routing numbers, tax records, medical details, login credentials, passwords, full dates of birth or copies of financial statements.
1. Scope
This Privacy Notice applies to information collected through mysaferollover.com and its appointment-request form. It does not replace a separate privacy notice, authorization, consent or disclosure provided by an insurance producer, agency, insurer, custodian, financial institution, professional adviser or service provider during a later application, recommendation, underwriting, account or transaction process.
2. Information collected
When a visitor requests contact, the website may collect the information the visitor chooses to provide, including name, telephone number, optional email address, service area, type of account or benefit, approximate rollover-amount range, principal protection goal, a short message and the visitor’s contact authorization. The form also records a submission timestamp and may transmit ordinary technical information such as Internet Protocol address, browser or device information and security-related headers.
The web server and hosting provider may automatically create access, error, security and mail logs. Embedded Vimeo content may receive browser or device information and may use cookies or similar technologies under Vimeo’s own privacy practices after the visitor chooses to load the player. This package does not include advertising pixels, cross-site behavioral advertising code or analytics by default.
3. How information is used
Information may be used to respond to an inquiry, schedule or prepare for an appointment, determine whether the inquiry is within the stated service area, route the inquiry to an appropriately licensed person, provide requested fixed indexed annuity or rollover-related information, document contact authorization, maintain security, prevent abuse, troubleshoot delivery, improve website operations, comply with legal or regulatory duties and preserve records reasonably related to the inquiry.
Submitting information does not create a professional relationship, determine suitability, authorize a transaction or guarantee an appointment, recommendation, product, approval or issuance.
4. Calls, texts and email
When a visitor checks the authorization box and submits the appointment form, the visitor authorizes My Safe Rollover and an appropriately licensed professional responding to the inquiry to contact the visitor at the supplied telephone number or email regarding appointment scheduling, retirement rollovers and fixed indexed annuity insurance products. Contact may include calls or text messages. Consent is not a condition of purchase. Message and data rates may apply.
Do not provide a telephone number or email address the visitor does not control or lacks permission to use. A visitor may ask not to receive additional ordinary marketing text messages by replying STOP or telling the caller. Transactional, compliance, servicing or legally required communications may be treated differently under applicable law.
5. How information may be shared
Submitted information may be shared with people and organizations reasonably necessary to respond or operate the service, including:
- personnel, licensed insurance professionals, agencies or marketing organizations involved in responding to the inquiry;
- website-hosting, email, telephone, security, form-processing and technology providers;
- professional advisers, auditors or vendors subject to appropriate obligations;
- insurance carriers, custodians or service providers when the visitor later requests or authorizes an application, illustration, underwriting review or transaction; and
- regulators, courts, law enforcement or other recipients when disclosure is required or reasonably necessary to protect rights, investigate abuse, respond to legal process or comply with law.
Appointment-form information is not sold for monetary payment by the website operator. Information later submitted in connection with an insurance application or transaction may be governed by additional producer, agency, carrier, custodian or vendor privacy notices.
6. Email delivery and local storage
The included website code is designed to send appointment information by email to the address configured in lead-config.php. It does not intentionally write form contents to a website database or local text file. Email servers, hosting systems, spam filters, backups and recipients may nevertheless retain copies or technical logs. The operator should configure access controls, mailbox security, retention rules and secure methods for any later exchange of sensitive documents.
7. Retention
Information may be retained for as long as reasonably necessary to respond, document the inquiry and authorization, maintain business and compliance records, protect against fraud or abuse, resolve disputes and meet legal, carrier or regulatory obligations. Retention periods may differ according to the type of record and later relationship. Some records may need to be retained even after a person asks that ordinary marketing contact stop.
8. Security
Reasonable administrative and technical safeguards should be used, but no website, hosting account, email system, telephone network or storage method is completely secure. Visitors should use HTTPS, a current browser and a trusted device. Sensitive documents should be exchanged only through a method specifically approved after an appointment is scheduled.
9. Cookies, Vimeo and future analytics
The featured video uses a click-to-load Vimeo player with Vimeo’s “do not track” parameter. The player is not loaded until the visitor chooses to play the video. Vimeo’s systems and policies remain outside the website operator’s control. If analytics, advertising, scheduling, chat or other third-party services are added later, this notice and any required consent mechanism should be updated before those services are activated.
10. Visitor choices and corrections
A visitor may avoid the form and call 505-990-8201. A visitor may request correction of ordinary contact information or ask that ordinary marketing contact stop. Requests are subject to reasonable identity verification and lawful record-retention requirements. This website does not promise a particular statutory privacy right where that right does not apply.
11. Children
This website is intended for adults seeking general retirement-rollover or fixed indexed annuity information. It is not directed to children, and children should not submit personal information. Do not submit information about a minor through the public appointment form.
12. External sites
Third-party sites, video players and external resources have separate terms, privacy and security practices. My Safe Rollover does not control those practices. Visitors should review the third party’s notices before providing information.
13. Changes to this notice
This notice may be revised when website features, vendors, practices or legal requirements change. The effective date above identifies the version included in this website package. Material operational changes should be reflected here before launch.
14. Contact
Privacy questions may be directed by telephone to 505-990-8201. My Safe Rollover operates by appointment and does not publish a public walk-in address on this website.
